PEBTF: Complete Spousal Attestation by July 31!

For Commonwealth employees hired on or after August 1, 2003: Take action to confirm your spouse is still eligible for PEBTF benefits!
You must confirm that your spouse is still eligible for PEBTF benefits. Refer to the PEBTF letter that is being mailed to you on May 21, 2025, and any reminders you may receive throughout the attestation period.
If you do not complete the annual attestation by July 31, 2025, your spouse will be terminated from PEBTF health benefits effective August 1, 2025.
You can complete the attestation in one of two ways:
- Online Attestation: This is the easiest way to complete the attestation. Just go to pebtf.org and click on the box at the bottom of the home page titled “Spouse Attestation” to begin. You will log in to the secure site and your answers from last year’s attestation will display. If nothing has changed, just select “yes.” The online form will populate last year’s answers and then follow any instructions. If there is a change, select “no” and you will be taken to the online form to complete. You will be instructed to provide any required forms. You can upload any forms at this time or come back later. File type must be .JPG (.JPEG), .TIFF (.TIF) or PDF. File size cannot exceed 4 MB. You will be kept informed all along the way. Once your attestation is processed, you will receive a confirmation letter in the mail, but feel free to check back online anytime to see if the attestation is complete.
- Fax, Email or Mail Completed Forms: Information on faxing, emailing or mailing your completed forms is included in the letter that is being mailed on May 21.
You will receive a confirmation letter when you have completed the attestation process.
